It is important to take your medication as prescribed. Do you carry your medicine with you while at school? Don't! Please bring all medications along with the Medication Authorization Form signed by your doctor and guardian/parent to the nurse. Do you have an inhaler? Do you have a doctor's note stating that it is okay for you to carry your inhaler while at school? Do you have a doctor's note for the use of an Epi-pen or a comparable pen? If not, please have your doctor fill out the Medication Authorization Form Request for Self-Administration of Inhaler/Epi-pen form. Reminder....please report all injuries (big or small) to the nurse, the principal, the assistant principal, your teacher, the secretary, the cook, the janitor, or any staff member. If reporting an injury to any staff member, please tell them to tell the nurse as well. We work as a team to keep you safe & healthy while at school.
